Upgrade drizzle-orm and drizzle-kit

drizzle-orm 0.39 -> 0.45 and drizzle-kit 0.30 -> 0.31. The migration tooling work in this epic should sit on current versions rather than upgrading afterwards.

• Read the changelogs between versions for breaking changes (query builder and pg driver areas especially)
• Run the full backend test suite after
• Keep this as its own commit so any behaviour change is bisectable
